The treatment form serves as an INGREZZA prescription and automatically enrolls patients into the Neurocrine Access Support program. The treatment form can be downloaded and faxed to the number at the top of the form or can be completed digitally via CoverMyMeds. Through CoverMyMeds, providers can:

• Electronically complete and submit the prescription through any in-network pharmacy
• Search for all INGREZZA network pharmacies by ZIP code, name, or city
• Electronically collect the prescriber’s signature and patient’s consent
